Congratulations on receiving a senior scholarship! Please complete the following information so that we can ensure the scholarship check will be sent appropriately. Scholarship checks will be made out to the college/university that you list below and the checks will be mailed to the home address at the end of June 2024. For a quick turn-around, we ask that you have all information submitted by Monday, June 3 and have mailed your thank you note to the donor by Monday, June 3 as well.
