  I hereby consent to participation by my child in Basketball Camp, hosted by Nouvel Catholic Central. I understand that this activity may take place away from NCC's grounds and that my child will be under the supervision of a designated adult while on or away from NCC's grounds. In consideration of my child being allowed to participate in this camp, I agree to indemnify and hold harmless NCC, any and all affiliated organizations, their employees, agents, representatives, including volunteer and other drivers, from any and all claims, negligence, arising from or relating to my child's participation in this sport. This indemnification does not apply to claims for intentional misconduct or gross negligence.
